It is a Sehgal Second Degree Rubric. Carried means the state of lifting (transportation) from one place to another, from present to the next (future) because the present is miserable not fit for staying.

He only request someone to be with him
He asks the Doctor to cure him, since he thinks this doctor has good experience and so trustful.
Carried uses "Desire" NOT "Want" which means fewer necessity.
Carry has a hide concept of moving, means the patient needs support for becoming or achieving a goal.
Patient request your treatment / Protection. So first he ask you then come to visit you

Ennui means just a change for the time being to break the boredom
Carried, desire to be means a permanent transportation from the present unwanted state to the next which is comfortable.
