Well, to begin with the patient is in a state where he is sure of nothing, means everything is merely a speculation. He starts constructing an idea that he has this bad, critical disease that someone he knew had as it started with similar symptoms that he has at present like prolonged cough, fever, blood stained sputum, pain or condition related to age, work or surroundings. The patient starts imagining that he has or is going to have a disease which has run in his family, without any substantial medical proof. His morbid fancy does not stop there. He imagines all the ways in which he would suffer as a result of the disease
